You need Adobe Acrobat Reader to open PDF files. Most computers already have this program - if yours doesn’t, just click on the icon above to get your free downlaod

1 Save the PDF form to ypur desktop (or other folder of your choice)

2 Complete the form offline - simply click the cursor into the first space, then use the ‘Tab’ key to move from box to box.

3 When your form is complete, click on ‘File’ then ‘eMail’ and send it to us at lespegouries@free.fr

Alternatively, you can print out the form and post it to us.
